Please complete each form and email or fax it with your payment to confirm your space. (fax# 843-353-2537). Please note, read, and sign our Kids Sea Camp/Family Dive Adventures waiver.
PADI elearning
eLearning (Click this link to start your PADI course today.) eLearning fees are paid directly to PADI. Once you choose and purchase your course selection, we receive a notice that you have started your course. Once completed, we will receive a notification and send you a certification Code.
You must bring your e-learning completion record to start your training. eLearning courses provided: Open Water Diver, Advanced Open Water Diver, Adventure, Underwater Digital Photo, Nitrox (Enriched Air), Rescue, Dive Master, and Instructor.
PADI Dive Training Forms and Waivers
You must bring these forms
These forms must be printed, completed, and brought to you for submission at the dive shop. If you do not bring these forms, you must complete them upon arrival before participating in any dive activity. Each link contains the PADI form needed for each PADI course you have requested.
Re-read the forms
We provide these forms for you to review, ask questions, and discuss any concerns or requirements with your doctor. PADI forms are mandatory for each student participating in PADI programs. Please download, read, and complete the forms. Bring the original forms with you, and turn them in at the dive shop upon arrival.
SASY: Kids must be age 5+ (1 set per child participating)
Seal Team: Kids must be age 8+ (1 set per child participating)
Jr. Open Water: for PADI Jr. Open Water course, 1 per student
Open Water: for PADI Open Water course, 1 per adult student.
ADULT Continuing Education form: is required for any adult participating in a PADI continuing education course and specialty classes.
CHILD Continuing Education form: is required for any child participating in a PADI continuing education course and specialty classes. The parent must complete it.
ADULT PADI KSC Waiver: 1 form per certified adult diver (18 or older)
CHILD PADI KSC Waiver :1 form per certified child diver must be filled out by a parent
Medical Form: This form is a statement informing you of the established safe diving practices for skin and scuba diving. Some destinations require this to be completed, and if you answer YES to any of the questions, you will need a physician to clear you to dive or participate in a course.
Standard Safe Diving Practices Statement of Understanding: Each participant must complete this form if he or she is in a course.
Parental Release for Emergency Treatment Consent V11 – 10088: The need for all certified diving kids: fill this form out for each child that is a certified diver not in a course. This form has already been included in the above-merged files for classes. This must be filled out by a parent or guardian, printed out, completed, and handed into the dive shop.
Other Forms and Waivers
COLLEGE CREDIT: Receive College credit for up to 15 PADI courses
GEAR RENTAL FORMS: For both Kids Sea Camp trips and Individual Family Dive Adventure trips
Equipment Rental Request Form: (If adults need to rent gear, or we provide gear for your kids, please include this form for sizing 1 per diver. Kids’ gear is included on all land-based destinations, and Adults’ dive gear is not included.)
Traveling with Kids
Single parents or parents with children with different last names. Parents traveling with children who are not their own outside the US need to check the immigration website of the country they are traveling to for the most up-to-date laws on entry and exit.
If you are traveling with kids outside the US, be informed: https://www.usa.gov/travel-documents-children. This is very important.
Passports and Documentation
To all travelers
All travelers must have a passport, sufficient funds to support themselves, and a return airline ticket to depart the USA. Travelers must have a passport valid for 6 months from the date of entry into any foreign country.
Travelers must ensure they have the proper documentation to travel into each country on their itinerary, as well as for re-entry and return to their country of origin. Please check with the appropriate consulate and your chosen airline carrier to ensure you have the proper documentation.
Always check your passport and visa requirements. US citizens may visit the US Department of State website at www.travel.state.gov for more information. The website also has information for consulates by country to assist with requirements.
Special requirements
There are special requirements for children leaving the US, and many countries have adopted special needs for the protection of children. If you are traveling with a child that is not your own, a single parent with a child, a child with a different last name, or traveling without the child’s natural parents, you will require documentation proving your right to travel with children. Since regulations vary by country, contact your consulate or embassy and visit the immigration site of the country you are traveling to for the requirements.The
Kids Sea Camp Reservation Office and staff will not assume responsibility for passengers who do not have the required documentation.

Regardless of our commitment to the highest safety standards in the dive industry, we highly recommend USI (Cancel for any Reason travel insurance) and DAN (Dive Insurance) if something unexpected happens on the island or a liveaboard.
Please note
